FIRST YEAR STUDENT-ATHLETES

Welcome first year student-athletes and transfers. Listed below is a check list you should use to be sure that you have filled out all of the paperwork required of you to initiate participation in intercollegiate athletics here at Goucher. The deadline for you to fill out and return all of these forms is July 15, 2009. The forms can either be faxed to 410-337-6576 or mailed to: Goucher Athletics, 1021 Dulaney Valley Road, Baltimore, MD 21204.

If you fail to complete and return this paperwork, you will not be able to begin practicing with the team when it begins its season. Should you have any questions, please do not hesitate to call our Athletics Office at 410-337-6383.

MEDICAL HISTORY: FIRST YEAR (4 pages)

Must be completed by first-year student-athletes (includes transfers).

EMERGENCY CONTACT INFORMATION:

Must be filled out by all student-athletes.

MEDICAL INSURANCE INFORMATION:

Must be filled out by all student-athletes and accompanied by a copy (front and back) of the health insurance card.

ACKNOWLEDGEMENT OF INSURANCE:  (2 pages)

All student-athletes must have the Acknowledgement of Insurance Requirement form signed by a parent or guardian.

SPORTS INFORMATION BIOGRAPHICAL FORM: FIRST YEAR  (2 pages)

Must be filled out by all student-athletes.

RETURNING STUDENT-ATHLETES
