Annual Requirements for Student-Athlete Eligibility
Welcome Gophers! Each year, student-athletes must complete NCAA compliance and institutional processes before participating in any Athletic activities, including strength and conditioning, practices, and competitions.
CHECKLIST OF TASKS
Before diving into the tasks, review the checklist of annual required items. Task instructions and links are included below.
All Student-Athletes (first-year, transfer, and returning)
Prior to arrival on campus
- Complete tasks in Medicat Portal (NEW students must create an account; instructions below)
- Complete the Physical exam within six months of the sports start date
Upon arrival to campus
- Health Review/Assessment with Goucher College Sports Medicine Team
- Sway Baseline Testing

Forms must be submitted electronically; they cannot be emailed, mailed, faxed, or hand-delivered.
Goucher's Medicat Portal
Who must complete: All student-athletes (requirements vary for new and returning athletes)
Task open: April 1st
Due date: July 1st

Returning Student-Athletes
- Update information if any changes from last year
- Complete a Physical Exam with a Medical Doctor using the Goucher Physical EXAM FORM (must be completed every two years)
- Exams must be within six months of your respective sports date
- Exams must be signed by a medical doctor only
- Forms must be uploaded to the Goucher Medicat Portal
- Complete annual Sports Medicine Form
Upon arrival to campus
- Health Review/Assessment with Goucher College Sports Medicine Team
- Sway Baseline Testing
NEW Student-Athletes (first-year, transfer)
STEP 1: CREATE AN ACCOUNT
Request a new account. Login using your Goucher username (example: filas001 do not include @goucher.edu) and password. If you have trouble signing in, try using an alternate internet browser (Chrome seems to work the best). If you continue to have difficulty, email nurse@goucher.edu
STEP 2: PHYSICAL EXAM
If you have not already done so, log in to your Medicat Portal.
- Complete "Acknowledgement of Risk & Consent to Treat" (address, phone number, class, etc.)
- Complete Medical History Questionnaire
- Upload Insurance Information (front and back)
- Upload a picture of your ID
- Complete your Physical using the Goucher Physical Exam Form
- Complete the required Sports Medicine forms for NCAA/IHSA Athletes.
- Complete Sickle Cell Information Sheet
- Fill out the form and upload your sickle cell information
STEP 3: REQUIREMENTS UPON ARRIVAL TO CAMPUS

Who must complete: All student-athletes
Task opens: Upon arrival
Testing will be completed on campus with our medical staff.
NCAA Eligibility Paperwork
Who must complete: All student-athletes
Task opens: Following on-campus meeting with compliance director
Due: Prior to participation
In addition to the annual review of Goucher's Student Handbook (Code of Conduct), Course Catalog, and Residence Life Policies, student-athletes must review the following policies and complete the required NCAA and Goucher compliance forms online through FrontRush before signing each one.

All NCAA Student-Athletes
- Review NCAA Division III Regulations Summary
- Review NCAA Banned Substances (list is subject to change)
- Complete NCAA Division III Student-Athlete Statement and Drug-Testing Consent
- Complete NCAA Misconduct Attestation Form
All NCAA Student-Athletes and Equestrian
- Review Goucher Social Media Guidelines
- Review Name, Image and Likeness Policy for student-athletes
- Complete Student Athlete Bio Form (returning students must complete as well)
- Complete Academic Consent Form
Mandatory Compliance Meeting
Who must attend: All NCAA student-athletes
Due: TBD - Coaches will provide the date, time, and location of all meetings
NCAA Amateurism Certification for New International Student-Athletes
Who must complete: All international college-bound student-athletes (first-year and transfers) who:
- Have a permanent residence outside of the U.S.
- Attended secondary school (high school) or postsecondary school outside the U.S. (excluding United States-based students studying abroad).
- Participated on a sports team or competes as an individual athlete based outside the U.S.
Task opens: April 1st
Due: Prior to the first day of class
Since final confirmation can take several weeks, request your amateurism certification early through the NCAA Eligibility Center for each Division III sport you plan to participate in. Amateur certification ensures you have not received payment for athletic participation other than the approved Name, Image, and Likeness (NIL) deals.
Login: Register or access your account here and choose Amateurism-Only Certification
- You can use an existing Academic or Amateurism Certification account.
- A fee is required, unless approved for a fee waiver
- Provide information about yourself, your education history, and your sports participation history. Submit supporting documentation as requested.
- Request your final amateurism certification for each sport to the Eligibility Center.
- Provide Goucher with your NCAA ID. The Eligibility Center may hold your certification until after you are added to Goucher's request list.
